Please arrive 30 minutes prior to your appointment to complete paperwork.

We encourage all pilots to log into: https://medxpress.faa.gov and fill out their portion of the 8500-8 form. When this is completed, please print out a copy and bring to your appointment along with the confirmation code. We will need this code to access your file. If you have been given a special issuance in the past, please be sure to bring this paperwork to your visit.  Also, bring all necessary information from your outside physicians so your certificate may be issued at time of service.
